About The Position

The Community Health Navigator advances the mission of Servicios de La Raza by providing education, screening, and linkage to care for individuals at risk for or living with HIV. This role exercises independent judgment and discretion in assessing client needs, coordinating care, and developing outreach strategies. Responsibilities include conducting community outreach, facilitating psychoeducational sessions, providing HIV testing and confirmatory testing coordination, and connecting clients to supportive services. By maintaining ongoing client engagement, collaborating with community partners, and ensuring accurate documentation, the Community Health Navigator promotes health equity, increases access to care, and strengthens community-based prevention and support networks.

Requirements

  • High School Diploma or equivalent required
  • Bilingual (Spanish)
  • Relevant years of experience will be considered

Nice To Haves

  • Bachelor’s degree in psychology, human services, social work, or other related field preferred

Responsibilities

  • Engage in community outreach education regarding PrEP, PEP, and HIV
  • Screen individuals for PrEP eligibility and interest, coordinate linkage to care, and connect clients to other supportive services
  • Plan, coordinate, and facilitate psychoeducational groups, learning sessions, and community outreach events
  • Conduct HIV testing for community members in non-clinical settings
  • Provide coordination and support for confirmatory HIV testing
  • Facilitate linkage to care for clients new to HIV services
  • Maintain ongoing contact to assess client needs through face-to-face visits, telephone, or email communication, using professional judgment to determine next steps
  • Collaborate and coordinate services with other HIV and PrEP service providers and community agencies
  • Assist with co-facilitation of psychosocial support services
  • Participate in targeted community outreach and events across a variety of settings
  • Provide internal and external resources as appropriate to client needs
  • Complete all necessary client and department documentation according to SDLR standards
  • Maintain confidentiality of all client-protected health information in compliance with HIPAA
  • Exercise independent judgment and discretion in prioritizing tasks, coordinating services, and responding to client and community needs
  • All members of the SDLR workforce are expected to support the agency’s community outreach, civic engagement, and recruitment efforts
  • Support the agency’s fundraising objectives, which may include tasks such as active fundraising, grant development, solicitation of donations, agency ambassadorship, informational outreach, and public speaking
  • Commitment to and experience with equity, cultural relevance, diversity, social justice, and disparity reduction practices with persons of color, agencies of color, and underserved communities
  • Ability to establish and maintain effective working relationships with clients, co-workers, agency partners, and the public
  • Other duties as assigned
