About The Position

We are a nonprofit dedicated to improving health equity and access to care for underserved communities across Staten Island. Through Community Health Worker-led programs, we help individuals and families connect to healthcare, social services, housing support, environmental health resources, and community-based care. We are seeking a compassionate and highly organized Community Health Worker to support Medicaid members enrolled in the SIPPS and PHS Medicaid Care Programs under the New York State Medicaid 1115 Waiver. In this role, you will serve as a trusted point of contact for participants, helping them navigate healthcare systems, access social services, address barriers to care, and stay connected to the support they need. This position is ideal for someone who enjoys working directly with communities, building relationships, solving problems, and making a meaningful impact in the lives of NY residents.

Requirements

  • High School Diploma or GED
  • Must be bilingual in Spanish & English
  • 1–2 years of experience in community health, outreach, care coordination, social services, case management, or a related field preferred
  • Experience working with Medicaid members, underserved communities, or vulnerable populations strongly preferred
  • Strong communication, relationship-building, and problem-solving skills
  • Highly organized with strong time management and attention to detail
  • Comfortable managing a high-volume caseload in a fast-paced environment
  • Comfortable conducting outreach, community-based work, and home visits across Staten Island
  • Proficient with Microsoft Office and electronic case management or documentation systems
  • Able to complete accurate and timely documentation

Nice To Haves

  • Community Health Worker certification or willingness to obtain certification
  • Experience with asthma programs, environmental health services, housing advocacy, chronic disease management, or Medicaid Waiver programs
  • Knowledge of NY healthcare, social service, and community resource systems

Responsibilities

  • Conduct outreach to eligible Medicaid members and support program enrollment
  • Complete participant screenings, assessments, and care plans
  • Manage an active caseload of at least 50 participants per month
  • Provide ongoing follow-up, care coordination, health education, and resource navigation
  • Help participants address barriers related to healthcare access, housing, food insecurity, transportation, benefits, and other social needs
  • Coordinate referrals with healthcare providers, social service agencies, managed care organizations, vendors, and community partners
  • Support environmental health and housing-related services, including home assessments, asthma remediation, and vendor coordination
  • Advocate for participants when challenges arise with care, benefits, housing conditions, or service access
  • Accurately document participant interactions, referrals, services, and outcomes in case management systems
  • Participate in supervision, team meetings, trainings, quality assurance activities, and program reporting
