About The Position

The YMCA of the Twin Tiers is partnering with Western New York Integrated Care Collaborative to help address the health care needs in two of the communities we serve - Allegany and Cattaraugus Counties. WNYICC is the Community Care Hub of a Social Care Network of local, trusted community-based organizations (CBOs) providing social care programs and services and Allied Partners (including healthcare and behavioral health providers). This position supports the work of the Y, a leading nonprofit, charitable organization committed to strengthening community through youth development, healthy living and social responsibility. The Community Health Coordinator leads the YMCA’s social care and population health initiatives, including all programming related to the NYS Medicaid 1115 Waiver. This role oversees and implements screening and navigating referral services, evidence-based health programs, and older adult wellness activity efforts, while cultivating partnerships and referral pathways with healthcare providers and community organizations. The coordinator supports and supervises volunteers, manages grants, and expands off-site, volunteer-driven programs that advance health equity across the service area. This is a grant-funded position, with the potential for continued employment based on performance, organizational sustainability, and future funding availability.

Requirements

  • Bachelor’s degree in public health, health promotion, community health, social work, nutrition, or a related field preferred.
  • Minimum of 2 years of experience in community health, non-profit management or equivalent experience, or experience in implementing evidence-based program models, including engaging specific or underserved populations, managing referral relations with healthcare providers, and implementing quality assurance and data collection protocols is preferred.
  • Successful experience providing and documenting care coordination, care management, community-based or public health services or, maintain an active Community Health Worker certification and Familiarity with NYS Medicaid 1115 Wavier is preferred.
  • Capacity to inspire and motivate others; Outgoing self-starter who takes the initiative to develop relationships with organizations, schools, and/or health care providers to identify individuals who qualify for HRSN services.
  • Proven track record of successfully meeting goals and key deliverables.
  • At least a year of supervisory and volunteer management experience preferred.
  • Strong organizational, interpersonal, and social skills with a demonstrated ability to collaborate with a variety of individuals from a wide range of professional and personal backgrounds.
  • Proficient computer skills and willingness to learn additional software applications.
  • Experience with HIPAA regulations is preferred.
  • Ability to interact effectively with diverse groups of people from all social and economic segments of the community to support building authentic, constructive relationships with others.
  • Ability to work independently and manage multiple priorities.

Responsibilities

  • Leads YMCA operations and compliance related to the NYS Medicaid 1115 Waiver, including screening, referrals, and documentation protocols.
  • Runs off-site screening and referral programs and actively engages in the community to build relationships, identify resources, and promote health services that meet the needs of underserved and high-risk populations.
  • Provide empathetic and culturally competent screening for social determinants of health to community members telephonically, virtually and/or in-person.
  • Process screens and referral intake daily including calls with potential program participants, documentation, and insurance/eligibility verification.
  • Receive, prioritize, monitor, and triage screenings and referrals that require service connections across community-based organizations and health care providers to ensure successful and timely connections between clients (community members) and the appropriate community partner.
  • Accurately document all services provided in the WNY Integrated Care IT platform and securely retain provision of service documents as required.
  • Builds and maintains a formal referral pipelines with healthcare providers, social workers, insurers, community partners, and government agencies. Maintain effective communication with community members and/or referring organizations to ensure acceptance and resolution of referrals.
  • Handles customer questions and concerns with excellent customer service, empathy, and patience.
  • Collaborates with WNY Integrated Care to produce data-driven performance reporting to support regulatory reporting obligations that will demonstrate value and build the evidence base on how HRSN services advance health outcomes and reduce health disparities.
  • Attends WNY Integrated Care Screening/Navigation Workgroup.
  • Demonstrates excellent customer service in all aspects of communication (phone, in-person, via email).
  • Works within WNY Integrated Care social care technology and transportation platforms.
  • Seek opportunities to enhance and simplify the user experience.
  • Abide by YMCA and WNY Integrated Care policies and procedures including those that support federal, state and local HIPAA and Fraud, Waste, Abuse regulations.
  • In compliance with federal law, all persons hired will be required to verify identity and eligibility to work in the United States in addition to background and Medicaid exclusions list clearances.
  • Seek opportunities to enhance and simplify the user experience including support for programs and events that target older adults and other priority populations.
  • Provide fundraising support through direct campaigning efforts for the Y’s Annual Campaign.
  • Carries out additional responsibilities as assigned
