Requirements

  • Bachelor’s Degree
  • De-escalation training within 6 months.
  • 2+ years working with community partners to advance health initiatives

Nice To Haves

  • Public Health, Public Relations, Social Work, or related field (Preferred Education)
  • Community Health Worker (CHW) certification or equivalent (Preferred Certifications, Credentials and Licenses)
  • Experience in healthcare or nonprofit settings (Preferred Experience)

Responsibilities

  • Facilitates partnerships that advance Adena Health’s community health goals across its ni county service area.
  • Supports outreach, collaboration, and coordination with community partners.
  • Incorporates a strong data focus.
  • Collects, analyzes, and interprets community health data to inform decision-making, track progress against Community Health Needs Assessments (CHNAs) and Community Health Improvement Plans (CHIPs), and ensure accountability in reporting.
  • Serves as an effective ambassador for Adena Health, strengthening relationships with local stakeholders and aligning community-based work with the health system’s mission and strategic priorities.
