Requirements

Bachelor’s Degree
De-escalation training within 6 months.
2+ years working with community partners to advance health initiatives

Nice To Haves

Public Health, Public Relations, Social Work, or related field (Preferred Education)
Community Health Worker (CHW) certification or equivalent (Preferred Certifications, Credentials and Licenses)
Experience in healthcare or nonprofit settings (Preferred Experience)

Responsibilities

Facilitates partnerships that advance Adena Health’s community health goals across its ni county service area.
Supports outreach, collaboration, and coordination with community partners.
Incorporates a strong data focus.
Collects, analyzes, and interprets community health data to inform decision-making, track progress against Community Health Needs Assessments (CHNAs) and Community Health Improvement Plans (CHIPs), and ensure accountability in reporting.
Serves as an effective ambassador for Adena Health, strengthening relationships with local stakeholders and aligning community-based work with the health system’s mission and strategic priorities.
