Serves as a link between the Parish Health Units (PHUs) and community programs and resources for the Bureau of Regional and Clinical Operations (BRCO). This role involves meeting clients in regional PHUs or other community locations to conduct needs assessments and help them set goals. The Coordinator will make regular follow-up calls and in-person visits, motivating clients to meet their identified goals. They will also assist clients with social issues such as homelessness, hunger, and employment, and help them make follow-up appointments and complete applications for Medical Assistance and SNAP. Additionally, the role requires collaboration with team members to create and maintain a directory of community resources, attendance at community engagement events, detailed documentation of client encounters, preparation of reports, and participation in scheduled meetings. Other tasks may be assigned.
