About The Position

The Community Habilitation Support Professional ensures high-quality service delivery that promotes person-centered, meaningful leisure skills, independence skill building, advocacy, and well-being for people served and their families. The role works with people receiving services and their families to develop and implement programs that enhance quality of life, while coordinating with community organizations, service providers, and stakeholders to build networks of support. The position also provides transportation and maintains required documentation, training, certifications, and regulatory compliance.

Requirements

• High school diploma or GED.
• At least one year of experience in community habilitation, respite services, or related fields, with a strong understanding of the needs of people with intellectual or developmental disabilities.
• Knowledge of advocacy and recreational resources and systems.
• Ability to work a flexible schedule, including weekends and evenings.
• Use of a personal vehicle and at least the minimum car insurance required by agency policy.
• Valid NYSDL.

Responsibilities

• Work collaboratively with people receiving services and their families to develop and implement programs that promote person-centered meaningful leisure skills, independence skill building, and well-being.
• Advocate for the needs of people receiving services within People’s Arc, with outside human services providers, and in the broader community.
• Collaborate with community organizations, service providers, and stakeholders to enhance the network of support for people on the caseload and their families.
• Provide transportation to leisure activities, medical appointments, and other destinations as needed.
• Ensure timely collection and high quality of Community Habilitation notes and other required documentation, including vehicle safety requirements and mileage submissions.
• Maintain all trainings and certifications.
• Ensure compliance with state and federal regulations, organizational policies, and best practices in service delivery.
