About The Position

The Direct Support Professional for the Day Habilitation program assists and supports participants in pursuing their individual goals, recreational activities, and community involvement while maintaining their personal comfort. This is a part-time, day-shift position located at Mall Drive in Iowa City, Iowa.

Requirements

At least 18 years of age.
High school diploma or equivalency.
Ability to read, write, and follow a written Individual Plan of Care.
Effective verbal, written, and interpersonal skills.
Ability to maintain professionalism and confidentiality with clients, staff members, agency leadership, and outside stakeholders.
Basic to intermediate computer skills.
Valid driver’s license, auto insurance, and reliable vehicle.

Nice To Haves

Previous experience in direct support services or a caregiver role is preferable.
Passion for supporting others while respecting individual differences.

Responsibilities

Interact with clients to support completion of daily goals described in each client’s Individual Service Plan (ISP).
Provide necessary support for individuals to participate in recreational activities and facilitate community involvement.
Assist with administering medications that are ordinarily self-administered, according to company policy.
Monitor and report observed changes in clients’ conditions or needs to the supervisor.
Document care provided by entering required information into client records.
Promote personal safety and maintain a safe environment for clients by observing infection control practices.
Communicate effectively with interdisciplinary team members through verbal reports and staff meetings.
Maintain confidentiality in all aspects of the position.
Attend in-service programs and training to meet compliance requirements.
Perform other related duties and responsibilities as deemed necessary.
