About The Position

The PurposeCare CCP Program Manager is a self-starter and highly detail-oriented leader responsible for managing, training, directing, executing, and continuously improving the operations of the CCP Program. This role coordinates and oversees the CCP Program to ensure operational efficiency, compliance, and quality service delivery. The CCP Program Manager possesses in-depth knowledge of Illinois CCP Program policies, procedures, and regulations related to local and state municipalities.

Requirements

  • High School Diploma or GED required
  • Minimum of five (5) years of progressive experience in business administration, preferably within healthcare, with demonstrated understanding of the healthcare environment
  • Minimum of five (5) years of experience managing and leading teams across multiple locations
  • Understanding of healthcare laws, regulations, and policies at the state, federal, and local levels
  • Strong leadership skills with demonstrated empathy, integrity, and accountability
  • Intermediate to advanced proficiency in Microsoft Office Suite, particularly Excel
  • Exceptional interpersonal, communication, organization, and multitasking skills
  • Ability to prioritize effectively and allocate resources to support short- and long-term goals
  • Ability to work variable schedules and participate in on-call responsibilities as needed

Nice To Haves

  • Bachelor’s Degree preferred
  • Strong understanding of Medicaid regulations preferred

Responsibilities

  • Overseeing the accuracy of personnel records
  • Entering mileage, show-up times, and trip fees
  • Managing the addition of caregivers into ClearCare (WellSky)
  • Managing caregiver activity, including terminations within the system as needed
  • Maintaining personnel compliance including TB, CPR, Licenses, and Evaluations
  • Serving as a general point of contact for caregivers
  • Coordinating with other agency leaders when waiver caregivers need to support private pay services
  • Verifying all Start of Care (SOC) documentation is accurate, including Physician information, SSN and insurance information, proper document uploads to the patient chart, and timely addition of ADLs
  • Conducting and reviewing all supervisor visits
  • Ensuring satisfaction surveys are completed timely and uploaded to the chart
  • Reviewing all schedules to ensure information is accurate at the start of services
  • Reviewing authorization utilization on the 1st and 15th of each month
  • Verifying scheduled hours are aligned appropriately against authorizations
  • Partnering with nursing teams to ensure respite cases have required orders for timely scheduling
  • Verifying the number of closed visits matches what is documented in ClearCare
  • Sending notices to caregivers regarding EVV non-compliance
  • Coordinating with the Regional HR Manager as needed
  • Training new administrative staff on CCP Program expectations
  • Conducting weekly and monthly meetings as necessary
  • Managing and resolving client and caregiver concerns
  • Addressing billing issues as needed
  • Serving as backup support for Case Manager, Scheduler, and Staffing functions
