The Oregon Department of Human Services’ Aging and People with Disabilities program is seeking a Community Based Care Surveyor (Client Care Surveyor) for its Community-Based Care team in Tigard, Oregon. The role supports person-centered services that promote safety, independence, and well-being for older adults and people with disabilities, while honoring choice, cultural preferences, and dignity. The surveyor conducts state licensing surveys of long-term care facilities and evaluates conditions for potential non-compliance. The work includes reviewing facility history and records, observing residents’ care and living conditions, interviewing residents, families, and staff, and inspecting facilities, including kitchens and food-preparation areas. The surveyor documents findings using specified computer software and applies state rules, guidelines, and professional judgment. The position combines community-based work with office work, with very limited work-from-home opportunities. The primary office is in Tigard, where training and trial service take place during the first nine months. The role requires regular, often overnight, travel Monday through Friday to counties across Oregon, for up to two weeks each month. Work takes place in busy and varying long-term care settings, may involve stressful or potentially trauma-inducing interactions, and requires mandatory reporting of abuse and neglect.
