The Clinical Pharmacist (PRN) will provide shift coverage in the inpatient pharmacy, primarily between the hours of 6:30 a.m. and 11:00 p.m. This role involves maintaining utmost confidentiality, adhering to hospital policies, and demonstrating ethical business practices. The pharmacist will work closely with pharmacy personnel to ensure safe and efficient patient care delivery, serving in a professional capacity in inpatient care areas. Key responsibilities include monitoring patients for optimal therapeutic outcomes, ensuring the appropriateness of medication doses, identifying and intervening in drug therapy problems (such as allergies, drug interactions, therapeutic duplication, and adverse drug reactions), and documenting interventions. The role also involves verifying the appropriateness of drug selection, initiating drug therapy when necessary, suggesting cost-effective alternatives, and evaluating the need for non-formulary medications. The Clinical Pharmacist will ensure departmental policies and procedures are followed, medication orders are processed promptly, and dispensed products meet quality guidelines. They will also ensure patients receive the correct medication, dose, route, and regimen, and that doses are filled and delivered within established guidelines. Accurate and efficient entry of medication orders into the computer system is required, along with initiating and processing medication incident reports and adverse drug reaction reports. The pharmacist will accurately complete Controlled Substance Inventory Records and perform assigned Controlled Substance Inventories, correcting any identified problems. Communication is key, including answering phone calls promptly and politely, communicating patient information to on-coming staff for continuity of care, and acting as a liaison between the pharmacy and medical/nursing staff. The role includes supervising pharmacy technicians and support staff to promote efficient workflow. Additionally, the Clinical Pharmacist must complete annual aseptic technique certification, accurately process outpatient prescriptions with patient counseling, maintain investigational drug study records, and actively participate in the education and training of new pharmacy employees, students, residents, and interns. They will also participate in ongoing self-education to meet CE credit requirements for license renewal and engage in departmental data collection and reporting for Drug Use Evaluation, Quality Assurance, and Quality Improvement activities. Routine checks of communication channels like email and mailbox are expected, along with performing other assigned duties.
