A caregiver in this role will work PRN supporting both decentralized and centralized pharmacy operations at South Pointe Hospital. This Clinical Pharmacist will primarily support Internal Medicine services by rounding with the interdisciplinary care team, reviewing and optimizing medication therapy, collaborating with providers and nursing staff, and providing clinical pharmacy support for hospitalized patients. Additional responsibilities include assisting with centralized operations as needed and supporting pharmacy education through precepting pharmacy students and learners. This is a PRN position. A caregiver in this role will work days/evenings including 1 holiday per year and every 5th weekend.

Requirements

  • PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy.
  • Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
  • Current Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross certification and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire are required for individuals required to administer immunizations.
  • Current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire, is required for individuals required to respond to adult or pediatric cardiopulmonary arrest events.

Nice To Haves

  • Board certification.
  • Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred.
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area

Responsibilities

  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers, and/or patients.
