About The Position

Thousand Branches Wellness is seeking an experienced Clinical Outreach Liaison for its outpatient mental health services for adolescents and adults in Chicago, Illinois. Through clinically informed community outreach and referral engagement, the role executes outreach activities and helps referral partners identify appropriate levels of care. The Liaison serves as a clinical resource in the community and helps bridge gaps between providers and treatment services. This is a field-based, externally focused position. It does not include responsibility for internal program operations, staffing, or clinical service delivery. Evening and/or weekend hours may be required as needed for community events.

Requirements

Bachelor's degree required.
One to three or more years of experience in behavioral health; clinical, case management, or utilization review experience preferred.
Strong understanding of behavioral health diagnoses and levels of care, including IOP, PHP, and residential care.
Established relationships within the behavioral health community.
Experience with CRM or referral tracking systems.
Strong ability to translate clinical knowledge into practical guidance for referral partners.
Consistent and reliable execution of outreach activities.
High level of organization and follow-through.
Comfort working within established outreach direction and priorities.

Nice To Haves

Master’s degree in a behavioral health field preferred.
Active licensure or registration strongly preferred.

Responsibilities

Execute assigned outreach plans, focusing on clinically appropriate referral development.
Build and maintain relationships with referral partners, including therapists, psychiatrists, hospitals, and community providers.
Conduct regular in-person outreach, meetings, and follow-ups to maintain a strong community presence.
Collaborate with Admissions and clinical teams to support appropriate and timely referral transitions.
Track outreach activity and referral trends to support consistency and follow-through.
