About The Position
Seacrest Village is seeking a Clinical Nutrition Services Coordinator to support resident nutrition and food-service programs at its Encinitas, California location. The role conducts nutritional assessments, develops individualized care plans, monitors residents’ nutrition and dietary needs, and collaborates with the Interdisciplinary Team. It also oversees menu and diet-order review, dining and food-service quality, staff education, documentation, quality improvement, and regulatory compliance. Requirements
- Able to pass a pre-employment physical and TB test.
- Obtain fingerprint clearance as required by licensure.
Responsibilities
- Conduct nutritional assessments and develop individualized care plans for new admissions, routine reviews, and changes in resident condition.
- Monitor resident nutritional status, weight trends, and dietary needs, and recommend appropriate interventions.
- Collaborate with the Interdisciplinary Team and participate in MDS assessments, care conferences, and resident care planning.
- Lead weekly meetings and maintain accurate nutritional documentation and resident records.
- Review and approve menus, therapeutic diets, diet orders, and meal services to ensure accuracy and compliance.
- Oversee dining and food-service operations to maintain quality, sanitation, safety, and resident satisfaction.
- Provide nutrition education and training to culinary, nursing, and other staff.
- Partner with Culinary Leadership to maintain department policies, improve operations, and ensure regulatory compliance.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities, monitor outcomes, and implement improvements.
- Support regulatory surveys and ensure compliance with CMS, OBRA, California Title 22, CDPH, and facility standards.
