About The Position

This role focuses on establishing relationships and facilitating communication with case managers, physicians, payors, patients, families, and referral sources to generate referrals and drive patient admission volume in the inpatient rehabilitation unit. The Clinical Navigator will ensure appropriate patient selection through clinical chart assessments and on/off-site evaluations in collaboration with physicians. Additionally, they will provide in-service and educational opportunities for healthcare professionals, serve as an educational resource for patients and families, and contribute to the development and implementation of business plans for the rehab unit.

Requirements

  • Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date.
  • Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Florida Board of Physical Therapy Practice obtained prior to hire date or job transfer date.
  • Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Florida Board of Occupational Therapy obtained prior to hire date or job transfer date.
  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Drivers License preferred.

Responsibilities

  • Establish relationships and aid with communication to case managers, physicians, payors, patients, families and referral sources in order to generate referrals and drive patient admission volume in the inpatient rehab.
  • Ensure appropriate patient selection through clinical chart assessments during on/off-site clinical evaluation of patients in collaboration with the physician.
  • Complete in-service and educational opportunities for physicians, case managers, interdisciplinary members and other professionals.
  • Serve as an educational resource to patients, family members and caregivers.
  • Participate in the development and implementation of business plans for the rehab unit and contribute to the development and maintenance of referral source promotional information.
