About The Position

The Clinical Liaison coordinates care needs for assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum. The role partners with patients and their primary physicians to create customized care plans based on individual needs and preferences, and collaborates with the broader care team to support timely, efficient, and effective treatment.

Requirements

• Academic degree in nursing.
• Two years of nursing experience.
• Currently licensed, certified, or registered to practice the profession as required by law, regulation in the state of practice, or policy.
• AHA BLS.

Responsibilities

• Coordinate the care needs of assigned patients and develop comprehensive plans to manage care delivery across the patient care continuum.
• Partner with patients and their primary physicians to develop customized care plans based on individual needs and preferences.
• Collaborate with physicians, nurses, allied health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion.
• Advocate for the patient.
• Balance care needs and financial considerations to ensure efficient and effective treatments are achieved.
