About The Position

The Multi Visit Patient Program (MVPP) Clinical Manager drives clinical services for a specialized care management program designed to improve utilization and health outcomes for identified multi-visit patients in BMC’s Emergency Department. Working collaboratively with leadership and providers across the health system—including the Emergency Department, Population Health, Psychiatry, and the Grayken Center for Addiction—the role brings expertise in behavioral health, social service resources, and emergency services. The Clinical Manager also engages internal and external community partners to strengthen care coordination and available resources. The position provides direct patient engagement, care coordination, crisis planning, and resource connection for high-risk patients, and recruits, supervises, coaches, and mentors BSW, MSW, and MPH students who provide patient care and administrative support. The role uses patient-centered and harm-reduction approaches, supports transitions of care and case conferencing, and completes documentation and data tracking in compliance with BMC and MassHealth requirements. It also contributes to program administration, analysis, and strategy. This is a full-time, on-site position, with most working time spent in the Emergency Department or inpatient units. Some flexibility in hours is required, including the possibility of shifts outside 9 a.m.–5 p.m., such as 7 a.m.–3 p.m. The position requires an MSW and at least five years of relevant experience.
