The Clinical Liaison builds and maintains referral relationships to support appropriate admissions to the hospital. The role coordinates intake and pre-admission screening, conducts clinical assessments—typically at referring facilities—to determine suitability for acute rehabilitation, and educates patients and families about services, levels of care, and participation expectations. The Clinical Liaison is the primary external contact for case managers, discharge planners, physicians, payors, and hospital partners. The position involves routine travel to referring facilities and community partners and close collaboration with admissions, nursing, therapy, and case management to support smooth transitions and payer authorization compliance. Success is measured by screening timeliness, conversion rates, and the quality of referral relationships.
