About The Position

Lamoille Health Partners is seeking a dedicated Clinical Care Coordinator to join their integrated Community Health Team (CHT) in Morrisville, Vermont. Requirements

Hold an active professional license or certification aligned with Vermont standards (e.g., mental health counseling, social work, or addictions).
Follow best practices outlined by your respective governing board.
Excel at building trust quickly, utilizing motivational interviewing, and practicing culturally sensitive care.
Understand family systems, community resources, insurance complexities, and how to successfully reduce barriers to care.
Thrive in an interdisciplinary environment.
Communicate seamlessly in an EMR.
Maintain absolute dedication to patient privacy and confidentiality.

Responsibilities

Use motivational interviewing and a whole-person approach to assess social, emotional, and financial factors, helping patients build actionable health goals.
Connect patients with essential community resources, including sliding fee scales, low-cost medications, transportation support, and post-hospital discharge care.
Provide brief individual counseling and utilize evidence-based tools (like CALM or CAMS models) to support patients navigating acute distress or barriers to external clinical care.
Serve as the bridge between individuals, families, and multidisciplinary providers to ensure seamless communication and continuity of care.
Participate in patient-centered medical home (PCMH) initiatives, team huddles, and quality improvement projects (like PDSA cycles) to continuously enhance patient care standards.
