About The Position

The Clinical Care Coordinator II provides patient care according to age-specific population needs and assists with management of patient care in a physician office setting under the direct supervision of a provider. This role is part of the FPG Nurse Triage Team and is based at FPG’s Centralized Access Center on University Parkway in Lakewood Ranch. It is a call-center position involving phone calls throughout the shift and does not provide direct patient care. The LPN takes calls from established FPG patients, follows symptom protocols, and offers guidance through strict adherence to established protocols to provide excellent care and real-time clinical guidance. The team collaborates with Clinical Excellence and Operations, providers, and provider-office care teams to support seamless patient experiences and effective communication.

Requirements

• Graduate of an accredited school of nursing or a diploma program.
• Basic Life Support/CPR certification, or obtain it within 30 days of hire or transfer.
• Florida Licensed Practical Nurse (FL LPN) license.
• Comfort working in a call-center environment and handling phone calls for the entire shift.

Nice To Haves

• One year of clinical experience in an outpatient physician office practice.
• Experience with electronic practice management and electronic health records.
• Demonstrated knowledge of electronic practice management and electronic health records software.
• Skill using Microsoft Office programs, including Word, Excel, Outlook, and PowerPoint.
