About The Position

CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening. Transporting Tribal Elders to medical appointments or other destinations as needed by the Senior Support Network. Offering assistance to the Elders as needed to get to their appointments. Follow the CPN CHR Transport policy.

Requirements

• Must have a High School diploma or GED equivalent
• Must have a good driving record and personal automobile insurance
• Must have very good communication and written skills
• Ability to work a varying schedule as needed.
• Sitting or standing or walking for long periods of time; occasional bending, squatting, kneeling, stooping; talking, hearing, and visual acuity
• Frequent lifting (up to 50 lbs)
• Occasional lifting (up to 75 lbs)
• Good driving skills

Responsibilities

• Provide Tribal Elders safe transportation to and from medical appointments or other destinations as required.
• Give assistance as needed to get the Elder to their appointment, this may include helping them with their assistive devices or with any paperwork they are given at their appointment.
• Document all mileage, addresses, and client signatures as required on Transport Logs.
• Promptly report any problems encountered with the SSN Supervisor.
• Provide friendly and courteous customer service.
• Perform other duties and projects as required by management.
