About The Position

AWARE is seeking a Child and Family Case Manager to provide support to children and families in the Anaconda community. The role helps clients gain access to medical, social, educational, vocational, and recreational services and supports their personal growth and participation in the community. The position is full-time, with starting pay of $22.00–$27.33 per hour. Requirements

Thorough understanding of human services, resources, and tools.
Ability to assess clients and families from a strengths-model perspective and develop a theoretical perspective.
Ability to work with a variety of people.
Strong oral and written communication skills.
A BS/BA degree in a human-services-related field is preferred; an equivalency may be considered based on documented work history, education, and volunteer experience.

Responsibilities

Provide case management services to clients.
Collect data from collateral sources.
Review relevant records and other pertinent data and resources.
Participate as a member of a treatment team in planning, formulating, implementing, and reviewing treatment, Personal Support Plans, or Individual Service Plans.
Provide services to identified children and families to help them gain access to medical, social, educational, vocational, and recreational services.
