About The Position
AWARE is seeking a Child and Family Case Manager to provide case management and support to children and families in Missoula, Montana. The role supports clients in becoming active and vital members of their communities while focusing on personal growth, and works as part of a compassionate, engaged team committed to providing high-quality, unconditional care. The case manager helps identified children and families gain access to medical, social, educational, vocational, and recreational services. The position is full-time and pays $22.00–$27.33 per hour to start.
Requirements
- A thorough understanding of human services, resources, and tools.
- Ability to assess clients and families from a strengths-model perspective and the skills, knowledge, and abilities to develop a theoretical perspective.
- Ability to work with a variety of people.
- Strong oral and written communication skills.
- A BS/BA degree in a human-services-related field is preferred; an equivalency may be considered based on documented work history, education, and volunteer experience.

Responsibilities
- Provide case management services to clients.
- Collect data from collateral sources.
- Review relevant records and other pertinent data and resources.
- Participate as a member of a treatment team in the planning, formulation, implementation, and review of treatment, Personal Support, or Individual Service Plans.
- Provide services to identified children and families to help them gain access to medical services.
- Provide services to identified children and families to help them gain access to social services.
- Provide services to identified children and families to help them gain access to educational services.
- Provide services to identified children and families to help them gain access to vocational services.
- Provide services to identified children and families to help them gain access to recreational services.
