About The Position
Cantex is seeking an LVN/LPN Charge Nurse to participate in and oversee patient care within the scope of LVN/LPN licensure at Skilled Nursing at Coronado at Stone Oak in San Antonio, Texas. The role involves observing, assessing, and reporting patient conditions and changes; documenting care in accordance with company policies and applicable guidelines; carrying out care plans; administering prescribed medications and treatments; and supervising CNA staff to support patients’ highest practical well-being. Requirements
- Graduation from an accredited nursing school.
- Valid LVN/LPN (Licensed Vocational Nurse or Licensed Practical Nurse) license.
Responsibilities
- Participate in and oversee patient care within the scope of LVN/LPN licensure.
- Observe, assess, and report patient conditions and changes.
- Document patient care in accordance with Cantex Clinical Policies & Procedures and state and federal guidelines and regulations.
- Complete EMR documentation relevant to patient assessment, evaluation, and changes in condition in accordance with nursing practices.
- Ensure implementation of care plans as directed.
- Administer medications and treatments as prescribed.
- Participate in assigning duties and supervise CNA staff in delivering appropriate clinical care to maintain patients’ highest practical well-being.
