Charge Nurse (LVN) (10p-6a)
About The Position
As a Licensed Vocational Nurse (LVN) at Forum Parkway Health and Rehabilitation, you will deliver compassionate, high-quality nursing care to residents. You will support residents’ health and well-being by administering medications, monitoring vital signs, and assisting with daily living activities under the supervision of registered nurses and physicians. You will collaborate with interdisciplinary teams to develop and implement individualized care plans that promote recovery and comfort. The role also involves advocating for residents and ensuring their needs and preferences are respected and met with dignity. Your work will directly affect residents’ quality of life and health outcomes.
Requirements
- Current and valid Licensed Vocational Nurse (LVN) license in Texas.
- Completion of an accredited vocational nursing program.
- Basic Life Support (BLS) certification.
- Demonstrated ability to perform nursing duties in a long-term care or rehabilitation setting.
- Strong communication and interpersonal skills.
Nice To Haves
- Experience working in a nursing home or rehabilitation facility.
- Additional certifications such as IV therapy or wound care.
- Familiarity with electronic health record (EHR) systems.
- Training in geriatric nursing or chronic disease management.
- Ability to speak multiple languages relevant to the resident population.
Responsibilities
- Provide direct nursing care to residents, including administering prescribed medications and treatments.
- Monitor and record vital signs, symptoms, and changes in patient conditions, and report significant findings to the healthcare team.
- Assist residents with activities of daily living, including bathing, dressing, and mobility support.
- Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans.
- Maintain accurate and detailed patient records in compliance with facility policies and regulatory requirements.
- Educate residents and their families on health maintenance and disease prevention strategies.
- Help ensure a safe, clean, and comfortable environment by following infection control and safety protocols.
