About The Position

Focused Post Acute Care Partners is seeking a Charge Nurse to provide nursing care to residents as prescribed by physicians, in accordance with nursing practice standards and regulations, and as directed by the Director of Nursing. The Charge Nurse leads or directs licensed and non-professional staff in providing direct resident care and support functions. A $6,500 sign-on bonus opportunity is available; ask for details. The role includes planning, organizing, and directing nursing functions for the unit; making assignments appropriate to staff skill levels; maintaining standards of patient care; implementing physician orders; and supporting infection-control policies. The Charge Nurse assesses residents, documents histories and vital signs, identifies and reports changes in condition, develops care plans, and communicates care information to residents, families, and the health care team. The position also requires responding appropriately to urgent or emergency situations, maintaining confidentiality and professional conduct, and using the PointClickCare electronic medical record system. The role requires a current LVN or RN license and CPR certification. Candidates must be able to understand and carry out verbal or written instructions in English, complete assignments in a timely manner, and remain flexible and positive during interruptions. The physical requirements include safely pushing, pulling, lifting, bending, kneeling, and reaching, including lifting up to 50 pounds, with or without reasonable accommodations.

Requirements

Current LVN or RN license.
Current CPR certification.
Ability to understand, remember, and carry out verbal or written instructions in English.
Ability to complete assignments in a timely manner.
Ability to remain flexible and positive despite intermittent interruptions.
Ability to safely push, pull, lift, bend, kneel, and reach, including lifting up to 50 pounds, with or without reasonable accommodations.

Responsibilities

Provide nursing care to residents as prescribed by physicians and in accordance with nursing standards, regulations, and the Director of Nursing’s direction.
Lead or direct licensed and non-professional staff in direct resident care and support functions.
Plan, organize, and direct nursing functions for the unit, making assignments appropriate to employees’ skill levels.
Maintain acceptable patient-care standards, identify problems, and guide personnel toward solutions.
Implement physicians’ orders accurately and promptly and support infection-control policies and procedures.
Obtain and document comprehensive nursing histories and perform basic patient assessments.
Record vital signs, note changes in condition, pursue further investigation as needed, and notify physicians and other appropriate parties of changes.
Develop care plans before the end of the duty period and use them as a basis for safe, therapeutic care.
Accept accountability for clinical care of assigned patients, including supervision of nonprofessional personnel.
Communicate with patients, families, and the health care team to carry out care-plan goals.
Perform technical skills required for the unit and respond appropriately to urgent or emergency situations.
Maintain confidentiality, professional conduct, positive public relations, and cooperative working relationships.
Seek guidance when encountering nursing procedures or responsibilities not yet mastered.
Use the PointClickCare electronic medical record for charting and follow workplace behavior and dress-code expectations.
