About The Position

Focused Post Acute Care Partners is seeking a Charge Nurse at Focused Care at Waxahachie in Waxahachie, Texas. The Charge Nurse provides resident nursing care as prescribed by physicians and in accordance with nursing standards and regulations, under the direction of the Director of Nursing. The role also leads or directs licensed and non-professional staff in delivering direct resident care and support functions. The position includes planning, organizing, and directing nursing functions on the unit; assigning work according to staff skill levels; assessing residents and responding to changes in condition; implementing physician orders; and developing care plans that support safe, therapeutic care. The Charge Nurse is responsible for maintaining professional relationships, confidentiality, and compliance with policies and procedures, and for documenting care using the PointClickCare electronic medical record. Requirements

Current LVN or RN license.
CPR certification.
Ability to understand, remember, and carry out verbal or written instructions in English.
Ability to complete assignments in a timely manner.
Ability to be flexible and positive amid intermittent interruptions.
Ability to safely push, pull, lift, bend, kneel, and reach, including lifting up to 50 pounds, with or without reasonable accommodation.

Responsibilities

Provide nursing care to residents as prescribed by physicians and in accordance with nursing standards, regulations, and direction from the Director of Nursing.
Lead or direct licensed and non-professional staff in providing resident care and support functions.
Plan, organize, and direct nursing functions for patients on the unit.
Make nursing assignments appropriate to employees’ skill levels.
Maintain acceptable standards of patient care and identify problems, guiding personnel toward solutions.
Create a working climate that supports personnel growth and job satisfaction.
Implement physicians’ orders accurately and promptly.
Support and enforce infection-control policies and procedures.
Maintain confidentiality and professional relationships with nursing staff, medical staff, other departments, and visitors.
Obtain and document a comprehensive nursing history from the patient or significant other.
Perform basic patient assessments and record vital signs, noting changes and pursuing further investigation as needed.
Assess changes in condition and notify the physician and other appropriate parties.
Develop a care plan before the end of the shift that gives incoming personnel sufficient information to care for the patient.
Use care plans as the basis for safe and therapeutic patient care.
Follow established standards, policies, and procedures when providing nursing care.
Accept accountability for clinical care of assigned patients, including supervision of nonprofessional personnel.
Communicate with patients, families, and other members of the health care team to carry out care-plan goals.
Perform technical skills considered basic to the care provided in the unit.
Respond appropriately to urgent and emergency situations and seek guidance on procedures or responsibilities not yet mastered.
Promote positive public relations and communicate tactfully in sensitive or emotional situations.
Use appropriate workplace behavior, follow the dress code, and document in the PointClickCare electronic medical record.
