About The Position
CLFC Healthcare and Communications is hiring a per diem Certified Nursing Assistant for Wayne County Nursing Home, a county-operated, 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has licensed nurses on every shift. The role is on-site and offers W-2 hourly pay of $25.00 to $28.00, with experience determining the rate and night and weekend shifts paid at the top of the range. Employees choose the shifts they want through the BRIDGE App, with no minimum hours; eight-hour and, when available, twelve-hour shifts are offered. Orientation is paid at the employee’s rate, and pay is weekly. The CNA provides residents’ day-to-day personal care and support, monitors and reports relevant information to nurses, and helps maintain a safe environment. The position also includes paid sick leave as provided under New York law and CLFC professional liability insurance while working shifts. CLFC arranges and pays for fingerprinting, immunizations, screening, and license verification. Additional programs include reimbursement for CPR renewal, support for continuing education and certifications, up to $1,500 per year in tuition assistance toward an LPN or RN program, and a $500 referral bonus for referring a nurse or aide. These programs are administered through CLFC University, with terms provided in writing before acceptance. A confirmed shift canceled by the facility two hours or less before its start is paid for four hours; cancellations with more notice are removed from the schedule. Overtime over 40 hours and all ten County holidays are paid at time and a half.
Requirements
- Current listing in good standing on the New York State Nurse Aide Registry.
- Current BLS (CPR).
- Six months of recent CNA experience.
- Able to complete the New York State Department of Health Criminal History Record Check and a drug screen.
- Current on immunizations required by New York for nursing home personnel.

Responsibilities
- Provide personal care, including bathing, dressing, toileting, and assistance with meals.
- Help residents move safely using proper body mechanics and lift equipment.
- Take and record vital signs, intake and output, and weights.
- Notice and report changes in residents’ conditions to the nurse.
- Keep residents’ environments clean and safe and follow infection control practices.
