About The Position
The Care Team Hospice is seeking a compassionate and dependable Certified Nursing Assistant to provide hands-on personal care and comfort to hospice patients while supporting their families. Working alongside an experienced interdisciplinary team, the Hospice CNA helps patients maintain dignity, comfort, and quality of life in their homes and care facilities. The role serves patients within approximately one hour of the branch and offers a schedule with no nights, weekends, or on-call shifts.
Requirements
- Active Certified Nursing Assistant (CNA) certification.
- Previous CNA experience is required.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
Nice To Haves
- Hospice, home health, long-term care, or skilled nursing experience is preferred.
Responsibilities
- Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
- Observe and document changes in patient condition and promptly report them to the supervising nurse.
- Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
- Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
- Complete accurate and timely documentation of patient care in the Homecare Homebase electronic medical record.
