About The Position

First Choice Home Health & Hospice is hiring a compassionate and dependable Certified Nursing Assistant to provide one-on-one personal care to patients in their homes. The role offers a consistent weekday morning schedule, meaningful patient relationships, and support from a clinical and office team.

The position is part-time, approximately 15–20 hours per week, Monday through Friday mornings. Availability beginning at 8:00 AM is essential to meet patients’ scheduled morning care needs. Work takes place in patient homes and residential care settings within the assigned service area.

Requirements

• Current active CNA certification.
• Availability Monday–Friday beginning at 8:00 AM.
• At least 18 years old.
• Valid driver's license.
• Reliable transportation.
• Dependable and punctual.
• Strong communication skills.
• Comfortable providing personal care in patients' homes.

Responsibilities

• Assist patients with bathing, grooming, dressing, and personal hygiene.
• Help patients with mobility and other activities of daily living.
• Support patient comfort, dignity, safety, and independence.
• Observe and report changes in patient condition.
• Communicate with nurses, patients, families, and the care team.
• Follow individualized plans of care.
• Complete accurate and timely documentation.
