About The Position

Provide supportive and personal services to home health patients while respecting their dignity and privacy. The role includes providing personal care and hygiene, following the instructions of a professional nurse or therapist, and observing and reporting relevant client conditions and safety concerns. The listed functions describe essential duties but are not an exhaustive account of all work requirements; additional duties may be assigned by supervisory personnel.

Requirements

High school graduation or equivalent preferred.
Ability to read, write, and follow directions.
Certified nursing assistant with proof of certification.
At least one year of full-time direct patient experience in an institutional setting, such as a hospital or nursing facility, or one year of full-time direct client care experience in an agency setting within the last five years.
Work positively and favorably with patients, families, and staff.
Demonstrate compassion, responsibility, and a cheerful attitude.
Listed on the nurse aide registry with no finding relating to client abuse or neglect or misappropriation of client property.

Responsibilities

Follow the instructions of the professional nurse or therapist when providing care.
Assist with hygiene, including bathing, oral care, and dressing.
Perform and accurately record measurements such as vital signs or intake/output as directed in care plans.
Observe and report safety hazards in the client’s home and significant observations about the client.
Attend staff meetings and complete 12 hours of annual in-services.
Participate in orientation of new employees.
Report patient complaints to the RN.
Maintain patient confidentiality and comply with HIPAA requirements and agency policies and procedures.
Follow written and oral instructions.
Report abnormal patient condition findings to the RN as observed and according to the care plan.
