About The Position
Champion Care is looking for an experienced Central Intake Admissions Coordinator to join our growing team! This position plays an important role in coordinating referrals, managing the admissions process, and connecting patients with the right level of care across our skilled nursing and rehabilitation facilities. This is an excellent opportunity for someone with experience in skilled nursing admissions, healthcare intake, hospital referrals, insurance verification, or patient placement who thrives in a fast-paced environment and enjoys making things happen.
Requirements
- Experience in skilled nursing admissions, central intake, healthcare admissions, patient placement, or referral coordination preferred
- Understanding of Medicare, Medicaid, managed care, and commercial insurance
- Familiarity with skilled nursing facilities (SNFs), rehabilitation services, and post-acute care
- Excellent communication, organization, and problem-solving skills
- Ability to manage multiple referrals and priorities simultaneously
- Experience with electronic referral platforms and healthcare systems preferred
- A proactive, detail-oriented professional who enjoys working with a team
Nice To Haves
- A nursing license is not required for this position. Clinical acceptance decisions requiring licensed judgment are handled by appropriate clinical personnel.
Responsibilities
- Review and process incoming referrals from hospitals and other healthcare providers
- Coordinate admissions across multiple skilled nursing and rehabilitation facilities
- Verify insurance coverage, benefits, and financial eligibility
- Assist with obtaining and tracking insurance authorizations
- Identify missing documentation and potential admission barriers
- Communicate with hospitals, discharge planners, case managers, and facility leadership
- Coordinate clinical referral reviews with appropriate nursing personnel
- Match accepted patients with appropriate facilities based on care needs and bed availability
- Monitor census, bed availability, and referral activity
- Maintain accurate records and timely updates within electronic referral systems
- Help convert accepted referrals into successful admissions
