About The Position
Enhabit Home Health & Hospice is hiring a Central Intake Coordinator to join our team in Alabama. The Central Intake Coordinator is responsible for regional coordination of patient referrals and ensures that adequate transition to home health services occurs. Hours: Monday - Friday, 8:00am - 5:00pm. The Central Intake Coordinator can work from one of these offices: Clanton Office: 2002 lay Dam Rd. Clanton, AL 35045, Hoover Office: 1 Chase Corporate Dr., Hoover, AL 35244.
Requirements
- Must have a high school diploma or equivalent.
- Must be organized, detail-oriented, and able to communicate effectively.
- Must have strong customer service skills.
- Must have demonstrated competency with computers, including advanced typing skills.
- Must be able to multi-task and problem-solve in a high volume, interactive environment.
Responsibilities
- Meets the productivity expectation to receive, document, and appropriately communicate referrals from physicians, facilities, and staff.
- Monitor electronic platforms to ensure timely initiation.
- Ensure the appropriate branch office receives referral information.
