About The Position

The Center Medical Director (CMD) leads the clinical team at an Oak Street Health center, providing high-quality patient care and supporting a positive workplace. The CMD models thoughtful primary care, mentors and coaches providers, and participates in organization-wide strategic initiatives. The CMD works closely with the Practice Manager and Outreach Manager or Director to advance organizational objectives, including excellent care, patient experience, growth, and a strong, inclusive workplace. The CMD helps implement organizational and regional priorities, communicates information to the care team, and escalates team concerns and ideas. The role supports provider onboarding, effective care-model implementation, evidence-based care, preventive care, and use of clinical guidelines. The CMD helps providers deliver care in a value-based, full-risk model, monitors center performance, and addresses root causes when targets are missed.
