About The Position

Metropolitan Family Services (MFS) is seeking a Targeted Case Management Specialist for its Merrionette Park Office. MFS is a community-focused organization dedicated to empowering families to learn, earn, heal, and thrive. This role involves providing community-based case management to clients with specific needs, including those at high risk of suicide or overdose, experiencing acute crises, facing critical life transitions (like homelessness or re-entry from incarceration), transitioning from institutional settings, or already enrolled in intensive programs. The specialist will gather client information, develop treatment plans, lead care planning, document services, monitor client status, and collaborate with multidisciplinary teams and community agencies to ensure coordinated care.

Requirements

  • Bachelor’s degree in a human services field or 2+ years' experience in the mental health field, social service or healthcare setting, required.
  • Must obtain and maintain IM+CANS certification, required.
  • Requires mobility to work with a community-based caseload.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
  • Experience working with diverse client populations with significant behavioral health needs; social service experience is preferred.
  • Punctual, reliable and dependable with the ability to work flexible hours (some evenings and weekends).
  • Ability to produce high-quality work independently.
  • Demonstrated commitment to social justice, and advocacy.
  • Satisfactorily pass all relevant screening and criminal background checks.
  • Driving for work required with transportation of clients preferred in a personally owned vehicle.

Nice To Haves

  • Bilingual in Spanish, preferred.

Responsibilities

  • Provides community targeted case management to CCBHC clients who meet specific high-risk criteria.
  • Gathers information for the IM+CANS identifying medical, social, legal, educational, and housing needs.
  • Develops specific goals integrated into the IATP (Individualized Assessment and Treatment Plan) related to identified targeted case management needs.
  • Actively leads care planning process that specify direct care resources to meet physical and psychosocial needs; by prioritizing problems and establishing mutually agreed upon goals specific to the client.
  • Documents the care and case management plans and other vital information in electronic health record after each contact and service.
  • Monitors daily alerts for participants entering an emergency room or inpatient hospital processes and other coordinated care interfaces, following up with clients and facilitating post-discharge appointments.
  • Participates as assigned in participant education, including development of materials, conducting presentations or supporting other team members in such efforts.
  • Works with multidisciplinary professionals and community agencies to provide a continuum of coordinated care to meet the complex needs of clients.
  • Participates in agency and site based required meetings and trainings.
