Requirements

  • Requires mobility to work with a community-based caseload.

  • Experience working with diverse client populations with significant behavioral health needs.
  • Punctual, reliable and dependable with the ability to work flexible hours (some evenings and weekends).
  • Ability to produce high-quality work independently.
  • Demonstrated commitment to social justice and advocacy.
  • Bachelor’s degree in a human services field or 2+ years' experience in the mental health field, social service or healthcare setting, required.
  • Must obtain and maintain IM+CANS certification, required.
  • Satisfactorily pass all relevant screening and criminal background checks.
  • Driving for work required with transportation of clients preferred in a personally owned vehicle.

  • Social service experience is preferred.
  • Bilingual in Spanish, preferred.

Responsibilities

  • Provides community targeted case management to CCBHC clients who meet specific criteria such as high risk of suicide or overdose, acute crisis episodes, critical transitions (homelessness, returning from incarceration), transitioning from institutional settings, enrolled in intensive programs, or receiving supported housing/employment services.
  • Gathers information for the IM+CANS to identify medical, social, legal, educational, and housing needs.
  • Develops specific goals integrated into the IATP related to identified targeted case management needs.
  • Actively leads the care planning process, specifying direct care resources to meet physical and psychosocial needs by prioritizing problems and establishing mutually agreed upon goals.
  • Documents care and case management plans and other vital information in the electronic health record after each contact and service.
  • Monitors daily alerts for participants entering emergency rooms or inpatient hospital processes and other coordinated care interfaces, following up with clients and facilitating post-discharge appointments.
  • Participates in participant education, including development of materials, conducting presentations, or supporting other team members.
  • Works with multidisciplinary professionals and community agencies to provide a continuum of coordinated care.
  • Participates in agency and site-based required meetings and trainings.
  • Other duties as assigned.
