The Targeted Care Manager (TCM) provides Clients in the Certified Community Behavioral Health Clinic (CCBHC) advocacy, outreach, education, and clinical services. The role requires skills in communication, cultural competence, training professional experiences, and education, with activities tailored to meet the unique needs of community members. The TCM ensures clients receive a coordinated range of client-centered services, linking them with services following the Social Determinants of Health (SDoH) and the Social Determinants of Mental Health (SDoMH). Responsibilities include completing client success plans, conducting periodic follow-ups, and timely entry of client data, units of service, case notes, incident reports, and engagement activities into the database. The position operates under the supervision of a case management supervisor, ensuring strengths-based, person-centered quality of care in a culturally and linguistically meaningful therapeutic environment.
