About The Position

Heartland Family Service is seeking a highly organized, analytical, and detail-oriented CCBHC Systems Coordinator to support and strengthen the agency's CCBHC data and quality improvement processes. This role ensures CCBHC programs consistently deliver high-quality, well-documented services, and that program data is accurately collected, understood, and used for decision-making. The position works closely with CCBHC leadership and operational teams to translate program priorities into effective workflows, reliable data, and meaningful reporting. This role also supports Zion Behavioral Health, Heartland Family Service’s CCBHC Designated Collaborating Organization (DCO) partner, to ensure aligned workflows, consistent data collection, and coordinated quality improvement efforts across both organizations. Regular travel between Heartland Family Service and Zion Behavioral Health locations is required.

Requirements

  • Bachelor's Degree in Data Analytics, Public Health, Social Work, Public Administration, or similar field
  • Three years of equivalent work experience, including behavioral health data analysis, KPI development, and workflow improvement.
  • Experience developing dashboards, reporting structures, and data visualizations.
  • Working knowledge of research methodology and data storytelling techniques.
  • Strong communication and presentation skills.
  • Strong computer skills.

Nice To Haves

  • Master's Degree preferred.
  • Experience working with Electronic Health Record (EHR) data preferred.

Responsibilities

  • Maps current workflows from referral and access through service delivery, documentation, and billing.
  • Identifies gaps, bottlenecks, duplication, and unclear handoffs between staff and programs.
  • Works with CCBHC leadership and operational teams to develop standardized workflows across locations.
  • Clearly defines roles, responsibilities, decision points, documentation requirements, and data collection within each workflow.
  • Establishes measures to evaluate access, completion of threshold services, timeliness, and operational efficiency.
  • Uses data to identify where workflows are working and where additional improvement is needed.
  • Inventories required CCBHC measures and organizational performance indicators.
  • Identifies where each data element originates, who is responsible for collecting it, how frequently it is collected, and where it is stored.
  • Creates and maintains a clear data map connecting frontline documentation to CCBHC measures and reporting.
  • Develops staff education to increase understanding of what data is collected, why it matters, and how frontline documentation impacts organizational reporting and outcomes.
  • Analyzes data collected across CCBHC programs and locations.
  • Develops concise reports and visualizations for leadership and external stakeholders.
  • Translates complex data into understandable findings and recommendations.
  • Helps demonstrate the impact of CCBHC services through data and outcomes rather than reporting numbers alone.
  • Travels regularly between Heartland Family Service and Zion Behavioral Health (CCBHC DCO partner) locations to support coordinated workflows, data collection, and quality improvement efforts.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the agency.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff, as well as staff at Zion Behavioral Health.
  • Is dependable and punctual regarding scheduling and attendance.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
  • Creates, maintains and shares as appropriate a dynamic self-care plan.
  • Strives to make connections between the agency and the larger community whenever possible to contribute to the agency's ongoing fundraising and friend-raising efforts.
  • Performs other program and project related duties as assigned.
