About The Position

Apalachee Center, Inc. is hiring a CAT Care Manager to provide case management services to youth and their families in Liberty and Franklin Counties. The role assists recipients in accessing needed medical, social, educational, and other services, with the goal of optimizing the functioning of youth with complex needs through coordinated, quality treatment and support services. Services and their frequency should reflect each recipient’s individual needs, goals, and abilities. The Care Manager assesses youth strengths and needs, including relevant family factors, and obtains information from family members and other providers when appropriate consent is available. The role includes conducting home visits before completing intake assessments unless an exception is discussed with a supervisor and documented in writing. The Care Manager develops and updates individualized service plans with youth and families, coordinates providers and natural supports, advocates for access to services, follows up on service effectiveness, and documents services and progress in the client record. Required documentation is finalized in Avatar within 48 hours.

Requirements

Bachelor’s degree from an accredited university or college, with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, health science, or a related human services field.
Current valid driver’s license.
Maintain Apalachee Designated Auto Driver status.
Maintain current ADM Targeted Case Manager Certification.

Nice To Haves

One year of full-time or equivalent experience working with children experiencing serious mental illness is preferred.
A bachelor’s degree in another field from an accredited university or college, together with experience working with children experiencing serious mental illness, is preferred.

Responsibilities

Complete comprehensive assessments of youth strengths and needs, including relevant family factors, and obtain information from family and providers as appropriate and with consent.
Update required assessments in accordance with Client Record Handbook guidelines, finalize documents in Avatar, and submit assessments within 48 hours.
Conduct home visits before completion of the intake assessment process to evaluate the client’s safety and well-being in their residence, unless an exception is discussed with a supervisor and justified in writing in the record.
Meet at least 85% compliance with home visit requirements on Clinical Reviews during the rating period.
Develop individualized service plans with measurable goals and objectives in partnership with youth and their families.
Complete service plans and reviews addressing youth and family factors, and assist with implementing plans while supporting family autonomy.
Update service plans at least every three months or when significant changes occur in the client’s condition, situation, or circumstances.
Help families establish natural support systems that provide ongoing support during and after program participation.
Collaborate with providers and natural support systems, advocate for youth and families, and help them build meaningful partnerships and address their needs at home, in school, and in the community.
Assist clients in accessing financial, transportation, employment, medical, social, educational, and other services.
Work with clients and their natural support systems to develop and implement service plans.
Follow up on services to determine their status and evaluate the effectiveness of service plans in enhancing community inclusion.
Document all services in the client record according to Client Record Handbook guidelines, finalizing documentation in Avatar within 48 hours of service delivery.
Write detailed progress notes linking case management efforts to service plan goals and describing client progress or lack of progress.
