About The Position

Wraparound Maryland is a nonprofit mental health services company with a holistic, person-centered approach. The Case Worker serves adults and families in Worcester County, advocating for clients and helping them access community resources and quality services. The role provides intensive, family-centered case management to support clients in achieving stability, permanent housing, employment, and self-sufficiency. Work is in-person and community-based.

Requirements

Bachelor of Arts or Bachelor of Science degree in social work, psychology, or a related field.

Nice To Haves

Extensive experience in human services is preferred but not required.

Responsibilities

Provide intensive case management services using a family-centered approach.
Help clients achieve goals that lead to stability, permanent housing, employment, and self-sufficiency.
Provide structure, direction, and service linkages to connect clients with community resources and overcome barriers to housing placement.
Complete an in-depth needs assessment and develop an Individual Service Plan (ISP) for each client, with specific goals and an action plan focused on permanent housing and self-sufficiency.
Meet bi-weekly with each client to track progress toward ISP and Participant Agreement responsibilities, providing direction and counseling as needed.
Connect clients with health, mental health, education, employment, housing, mainstream benefits, and parenting assistance services.
Monitor client attendance at on-site life skills, parenting, and employment-readiness classes.
Maintain accurate, up-to-date case records and notes documenting client progress and services delivered.
Develop positive relationships with providers to help ensure families can access quality services.
