About The Position

The Case Planner (Aftercare) supports AFG’s mission, and the goal to deliver quality case management services, meeting with program participants to complete intake, assessment, goal setting and discharge.

Requirements

  • Requires an associate or bachelor’s degree in social work, counseling, psychology, sociology or criminal justice and a minimum of 3-5 years of related experience
  • Requires basic computer software and Internet skills
  • This position requires regular driving. Incumbents must have reliable transportation, must possess a driver’s license and automobile insurance, and must have a good driving record.
  • Solid organizational and case management skills
  • Ability to set priorities, take initiative and meet deadlines
  • Good communication skills, diplomacy and tact
  • Good decision-making, conflict-resolution and effective listening skills
  • Ability to handle crisis, cope with job stress and maintain confidentiality

Nice To Haves

  • Experience working with youth who are juvenile offenders, homeless or involved in high-risk activities, such as: gangs, violence, drugs, truancy, sexual activity is strongly preferred

Responsibilities

  • Engage and deliver treatment services to program participants that support aftercare goal attainment (independence, safe/stable housing, education, employment, mental health, etc.)
  • Screen, assess, and process referrals for intake into the shelter program.
  • Research and develop community resources, make connections, and build relationships (employment, education, housing) in order to serve the needs of program participants.
  • Transport participants to initial appointments such as: employment interviews, apartment viewings, setting up daycare, obtaining necessary documents, etc.
  • Maintain contact with participants; conduct weekly home visits, monitor living situation, and ensure participant involvement in support groups. Complete quarterly progress reports.
  • Teach life skills (e.g., cooking, cleaning, organizing, shopping, paying bills, establishing a bank account, accessing transportation, etc.) to assist with a successful transition into the community.
  • Ensure discharge planning activities and docs are completed within 48 hours of discharge.
  • Provide direct care and supervision of shelter residents, as needed.
  • Perform other duties, as assigned.
