About The Position

The Case Manager provides services within Volunteers of America’s Case Management Model, working directly with clients to assess their strengths and needs, develop and implement service plans, connect clients with appropriate resources, provide counseling, and monitor progress. The role focuses on client engagement and service access while ensuring clients receive timely, professional, and comprehensive support to achieve successful outcomes. This position is part of Supportive Services for Veteran Families (SSVF), a community-based outreach program that identifies low-income veteran households and connects them with supportive services to promote housing stability and prevent homelessness. The position is located in Bronx, NY 10451.

Requirements

60 college credits plus two years of relevant experience, or a satisfactory combination of education and experience.
Demonstrated verbal and written communication ability.
Basic computer skills.

Responsibilities

Manage client intake and assessment to orient clients to the program and develop a comprehensive assessment of their service needs.
Develop an individual service plan with each client based on assessment findings and goals, and update the plan with the client as needed.
Conduct outreach to veterans enrolled in SSVF, supporting active participation and access to services, including through home visits or outreach in shelters as necessary.
Meet regularly with clients to help them obtain services and benefits that support their individual service plan goals.
Provide supportive counseling to strengthen clients’ ability to make appropriate life decisions.
Document case management activities according to agency and funder guidelines and procedures.
Collect service data and participate in evaluation and quality assurance activities.
Support outreach screening and assessment to identify veterans who may be eligible for SSVF services.
