About The Position

Avenues Recovery Center is hiring a Case Manager for its inpatient detox and residential treatment facility in Fort Collins, Colorado. The Case Manager advocates for clients, addresses barriers to treatment, manages client needs and accounts during treatment, and coordinates resources and discharge planning to support clients after treatment.

Requirements

• High school diploma or GED preferred.
• At least one year of experience in the treatment industry preferred.
• If in recovery, at least one year of sustained sobriety is required.
• Excellent communication, multitasking, and time-management skills.
• Strong problem-solving and critical-thinking skills.
• Warm, empathetic, and respectful personality.

Responsibilities

• Advocate for clients and address and resolve external barriers to treatment.
• Initiate case management for each client upon intake and address pressing needs.
• Oversee client accounts and ensure insurance remains active throughout treatment.
• Provide ongoing case management for the duration of treatment.
• Arrange post-discharge resources as needed, including housing, financial aid, legal aid, and vocational training.
• Facilitate comprehensive discharge planning, including continuing care, housing, and sober support.
