About The Position

The Case Manager provides services within Volunteers of America’s Case Management Model, working directly with clients to assess their strengths and needs, develop and implement service plans, connect clients with appropriate resources, provide counseling, and monitor progress. The role focuses on client engagement and access to timely, professional, comprehensive support to achieve successful outcomes. The position is part of Supportive Services for Veteran Families (SSVF), a community-based outreach program that identifies low-income veteran households and connects them with supportive services to promote housing stability and prevent homelessness. The role is located in the Bronx, NY 10451.

Requirements

60 college credits plus two years of relevant experience, or a satisfactory combination of education and experience.
Demonstrated verbal and written communication skills.
Basic computer skills.

Responsibilities

Manage client intake and assessment, orienting clients to the program and assessing their service needs.
Develop an individual service plan with each client based on assessment findings and goals, and update it as required.
Conduct outreach to veterans enrolled in SSVF, supporting active participation and access to services, including home visits or outreach in shelter settings as necessary.
Meet regularly with clients to help them obtain services and benefits needed to achieve their service-plan goals.
Provide supportive counseling to strengthen clients’ ability to make appropriate life decisions.
Document case management activities according to agency and funder guidelines and procedures.
Collect service data and participate in evaluation and quality assurance activities.
Support outreach screening and assessment to identify veterans who may be eligible for SSVF services.
