About The Position
SSM Health Rehabilitation Hospital, a joint venture between Select Medical and SSM Health, is seeking a dedicated Case Manager for their Inpatient Rehabilitation Hospital. Requirements
- Current Licensure per state guidelines in clinical or related discipline (RN Preferred) OR a Bachelor's or Master's in a health or human services discipline.
Nice To Haves
- Previous experience in Case Management and Discharge Planning preferred.
- CCM Certification Preferred.
- Acute Rehab experience preferred
- RN/PT/OT/SLP/RT/SW preferred
Responsibilities
- Coordination of health care decisions using a systematic approach to assure treatment plans that improve quality and outcomes.
- Coordination of care across the continuum.
- Promotion of cost-effective care within the allotted time frame.
- Assuring payments of hospital-based services meeting patient-related utilization management criteria.
- Implementation of safe and appropriate discharge plans.
- Assessment of the psychosocial needs of the patient and provision of intervention as part of the discharge planning process.
