About The Position

Medica's Social Work Case Managers provide a member-centric, evidence-based model of care across multiple products. Our Case Management program aims to serve the members with highest needs and help them navigate the health system. Through individualized assessments, case managers identify each member’s goals of care, coordinate services across various settings, and connect members with community resources that align with their needs. This holistic approach enables case managers to reduce the burden of illness for both individuals and their families while contributing to lower overall healthcare costs. This position involves collaboration with members and their families, caregivers, providers, and interdisciplinary teams. Performs other duties as assigned.

Requirements

• Bachelor's degree in Social Work.
• 5+ years of clinical/acute care experience.
• Current, unrestricted Social Work license/certification in the state of residence.
• Ability and commitment to obtain Certified Case Manager (CCM) within two years of hire.

Nice To Haves

• Master's degree.
• Clinical licensure.
• Certified Case Manager (CCM).
• Experience working with vulnerable and complex populations, including multiple age groups, ethnic and socioeconomic backgrounds provided in a clinical, home care or telephonic environment; direct case management experience strongly preferred.
• Experience and at ease working with various populations: multiple age groups, ethnic, socioeconomic backgrounds and a generalized level of understanding across specialty care areas.
• Experience managing multiple computer systems and tools.
