This role involves working with the elderly and disabled individuals who are at risk of entering a nursing home. The primary goal is to explore their options and alternatives to institutional care. As part of the Supports Coordinator Team, which includes a Social Worker and an RN, you will complete comprehensive assessments with a strong emphasis on Person-Centered Planning and participant direction. Many of the specific duties are mandated by the Michigan Department of Health & Human Services (MDHHS), Bureau of Aging, Community Living, and Supports (ACLS Bureau) Care Management Performance Standards.
