About The Position

This role involves acting as a mobile resource and primary advocate for patients across multiple facilities including St. Francis, St. Joseph, St. Teresa, and the Rehabilitation Hospital. The Social Work Case Manager will design person-centered care plans that address unique patient needs, cultural preferences, and personal goals. They will use clinical expertise to assess the intersection of emotional, psychological, and physical health, identifying areas for social work intervention. The role also includes championing patient rights within the medical system and collaborating with leadership and the care transition team to ensure effective execution and evolution of care plans.

Requirements

  • Licensed Clinical Counselor credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.
  • Licensed Counselor credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date. Or Master Social Worker credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.
  • High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
  • Case Manager credentialed from the American Case Management Association (ACMA).

Responsibilities

  • Act as a mobile resource and primary advocate for patients across St. Francis, St. Joseph, St. Teresa and the Rehabilitation Hospital, providing a consistent point of support between facilities.
  • Craft holistic care plans that don’t just address medical charts, but honor the unique needs, cultural preferences, and personal goals of every patient.
  • Use clinical expertise to assess the intricate intersection of emotional, psychological, and physical health, identifying exactly where and how social work intervention can change a patient’s trajectory.
  • Serve as a dedicated advocate within the medical system, ensuring our patients’ voices are heard, their rights are protected, and their care remains respectful and individualized.
  • Maintain a continuous, strategic dialogue with leadership and the care transition team to ensure care plans are effectively executed and evolve as the patient’s journey progresses.
