About The Position

DeCoach Recovery Centre provides comprehensive, compassionate care to people with substance use disorders, using evidence-based treatments tailored to clients’ needs and focused on long-term recovery. The Case Manager provides direct services to adults with substance use and mental health disorders, addressing individual needs through resource linkage, daily living skills development, advocacy, and support for personal recovery goals. The role includes visiting clients in local nursing homes, taking vital signs, administering drug screens, and arranging telehealth visits with providers. Services are primarily community-based, and the position may use flexible hours to best serve clients.

Requirements

• Graduate of an accredited college or university, or high school graduate.
• At least one year of experience in a behavioral health or related field.
• Active Ohio driver’s license.
• Direct knowledge of behavioral health issues, correct documentation procedures, and current therapeutic methods.

Responsibilities

• Provide case management to adults with substance use and mental health disorders under clinical supervision.
• Deliver primarily community-based services, meeting clients where they are; use a properly insured company vehicle.
• Use excellent communication skills to facilitate client care.
• Provide therapeutic services identified by the Treatment Team and specified in each client’s Individual Treatment Plan, including individual and group case management, community activities, facility visits, vocational training, and care coordination.
• Comply with DeCoach Rehabilitation Centre policies and procedures.
• Work with administrative and team managers to improve service quality through ongoing studies of existing services and needs assessments for improvement and revision.
• Visit clients at local nursing homes, take vital signs, administer drug screens, and arrange telehealth visits with providers.
• Provide resource linkage, activities of daily living skills development, advocacy, and support toward clients’ personal recovery goals.
