About The Position

JFCS is seeking a compassionate professional to provide case-management services to children receiving behavioral-health services across The Valley. The Child Case Manager also provides guidance and support to the families and caregivers of these youth as needed.

Requirements

Meets one of the following education and experience requirements: High School Diploma/GED and 4 years of Behavioral Health experience; A.A. Degree and two years of Behavioral Health experience; or a Bachelor’s Degree in Social Work, Psychology, or a related field.
Level 1 Fingerprint Clearance Card without restrictions.
Meets agency requirements for Motor Vehicle Record (MVR) report.
Reliable transportation.
Arizona driver’s license.
Arizona auto insurance.

Responsibilities

Provides weekly contact with assigned clients, including home and community visits and attending medication checks.
Assists youth in navigating discharge from treatment settings, finding services as clinically indicated, and reducing behaviors to improve stability in the home and community.
Coordinates services and collaborates with involved agencies, including the Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
Coordinates and facilitates Child & Family Team (CFT) meetings.
Collaborates with the clinical team to ensure Integrated Service Plans, CFT meeting notes, and required paperwork are completed accurately and in a timely manner.
