About The Position

A community healthcare facility near Boston, Massachusetts, is seeking a Case Manager to join its team. This is a part-time, permanent, direct-hire position in an inpatient setting with a day-shift schedule. The pay range is $40.00–$53.66 per hour. The Case Manager evaluates patient or client needs, reviews medical and social records, and develops tailored treatment or care plans. The role includes arranging safe post-hospitalization care, rehabilitation facilities, or home health services to help prevent readmissions. The Case Manager also advocates for client rights, navigates insurance pre-authorizations, and responds promptly to crises such as housing instability or abuse. The position requires qualification to independently complete assessments within the scope of practice of the candidate’s discipline, an active and unencumbered Massachusetts or Compact license, and at least two years of rehabilitation experience. Candidates must have or be eligible to obtain a CCM or ACM certification.

Requirements

Be qualified to independently complete an assessment within the scope of practice of the candidate’s discipline, such as Registered Nurse, Social Worker, Occupational Therapist, Physical Therapist, Speech Therapist, or Rehabilitation Counselor.
Hold an active and unencumbered Massachusetts license in the candidate’s discipline or a Compact License.
Have or be eligible to obtain a CCM or ACM certification.
Have at least two years of rehabilitation experience.
